ADULT GUARDIANSHIP OF THE PERSON

Adult Guardianship Services

Connect And Care LLC accepts adult guardianship of the person referrals for review, centered on individual rights, preferences, culture, and least-restrictive options. Every referral is reviewed for appropriateness and remains subject to court appointment when required.

Now accepting adult guardianship referrals for review.

A referral does not mean Connect And Care LLC has accepted the case or agreed to serve as guardian. Each referral is reviewed for appropriateness, conflicts, capacity, legal requirements, less-restrictive alternatives, and court appointment when required.

A court appoints a guardian separately for each person; nothing on this page creates guardianship authority or represents that Connect And Care LLC is licensed, certified, state-approved, or appointed in any case.

01 · Scope

What Adult Guardianship Means

Adult guardianship is a court-created legal relationship that may authorize another person to make some decisions for an adult when the court determines that guardianship is necessary and appropriate. It is not a default response to a diagnosis, disability, language difference, or family disagreement.

Disability, diagnosis, limited English, family disagreement, or turning 18 alone does not establish a need for guardianship. The adult’s rights, abilities, preferences, and less-restrictive alternatives must be considered. A court appoints a guardian separately for each person and defines the authority in the court’s order.

02 · Service boundary

Our Scope

Adult guardianship of the person only.

This referral intake is limited to adult guardianship of the person and does not include the following:

  • Minor guardianship or cases involving children.
  • Conservatorship, estate management, or financial decision-making.
  • Legal advice, legal representation, or a decision about incapacity.
  • Emergency response, crisis intervention, clinical treatment, or direct care.
  • Eligibility, benefits, housing, medical, or service-authorization decisions made by another responsible organization.
  • Any appointment or authority that has not been independently granted by a court.

03 · Responsibilities

What We Do

    • Listen to the adult’s voice, preferences, values, culture, and goals.
    • Explain the proposed guardianship role and court process in plain language.
    • Support communication with the adult and the people involved in the matter.
    • Consider less-restrictive alternatives and the adult’s existing supports.
    • Coordinate with the court, attorneys, providers, agencies, and chosen supporters as appropriate.
    • Help identify practical needs and connect the adult with relevant community resources.
    • Keep clear, timely records of decisions, contacts, and required follow-up.
    • Review whether the arrangement continues to reflect the adult’s rights and current needs.
    • Raise questions, concerns, and possible changes through the proper court and oversight channels.

04 · Rights-centered approach

Our Commitments

    • Treat each adult with dignity, respect, and patience.
    • Center the adult’s voice and support the greatest possible self-determination.
    • Use the least-restrictive option that can meet the person’s needs.
    • Communicate in plain language and honor language, culture, accessibility, and communication needs.
    • Include the adult and appropriate supporters in decisions whenever possible.
    • Protect privacy and share information only for a legitimate purpose and through an appropriate channel.
    • Be transparent about roles, limits, conflicts, and the difference between information and legal advice.
    • Document actions and remain accountable to the court and applicable oversight requirements.

05 · Referral review workflow

How the Process Works

  1. 1A person or organization submits an adult guardianship of the person referral.
  2. 2The request is reviewed for basic scope, safety, and possible conflicts of interest.
  3. 3Connect And Care LLC explains what this adult guardianship service could and could not cover.
  4. 4The adult’s voice, preferences, supports, and less-restrictive options are considered.
  5. 5If appropriate, the responsible parties follow the applicable court process.
  6. 6A court independently decides whether to appoint a guardian and identifies the authority granted.
  7. 7Any engagement would follow the court’s order, ongoing review, and accountability requirements.

A referral does not create guardianship authority. Only a court can appoint a guardian and define that guardian’s authority.

06 · Protections

Safeguards and Accountability

    • The adult’s rights, preferences, and participation remain central.
    • Court oversight and the limits of any court order are respected.
    • Confidential records and sensitive details are kept out of this public inquiry form.
    • Possible conflicts, dual roles, and undue influence concerns are reviewed before any engagement.
    • Questions or concerns can be raised through appropriate oversight and complaint channels.
    • This public page does not provide legal advice, decide incapacity, or replace an attorney, court, or emergency service.

Conflict policy

Connect And Care LLC would not accept or continue a matter when a conflict of interest, dual role, personal interest, or other circumstance could compromise independent judgment or the adult’s interests. A potential conflict would be disclosed and addressed before any engagement.
