DEVELOPING CFSS CONSULTATION SERVICES IN MINNESOTA
A clearer way forward.
Connect And Care LLC is a Minnesota human-services organization developing CFSS Consultation Services to help participants and families understand their choices, prepare for person-centered planning, and navigate the CFSS process with greater clarity and confidence.
Connect And Care LLC is not Minnesota DHS, a lead agency, or an FMS provider. Connect And Care LLC is developing CFSS Consultation Services and should not be represented as DHS-authorized, contracted, or enrolled unless that status is confirmed. This website provides general information and does not determine eligibility or authorize services.
Understand. Choose. Participate.
Information with room to breathe
Our purpose
Confidence begins with understanding.
CFSS can open meaningful choices, but the language and roles can feel like a lot. We create practical, respectful explanations that help people prepare for conversations and stay connected to what matters in everyday life.
The person remains the decision-maker. Our job is to make information easier to find, understand, and use.

Developing for the Future
DHS-contracted CFSS Consultation Services
Connect And Care LLC is not currently a DHS-contracted CFSS Consultation Services provider. The consultation services described elsewhere on this website remain a future direction, separate from the five education and training services available now.
Connect And Care training is supplemental and does not replace required Minnesota DHS/MHCP training, certification, enrollment, testing, orientation, or other provider requirements. Nothing here indicates DHS approval, endorsement, authorization, contracting, or enrollment.

Care is more than a service.
It is a person’s routines, relationships, language, culture, goals, and sense of home. CFSS conversations are stronger when all of those parts can be heard.
We use culturally responsive, accessible language and leave room for each person to decide who belongs in the conversation.
Questions are useful
You do not need to know the right words before you begin.
Tradeoffs deserve honesty
Every model has responsibilities. Clear information supports an informed choice.
The person stays central
Planning starts with what matters to the participant—not a template.
