Connect And Care LLC’s mission is to help older adults, people with disabilities, and people with limited English proficiency access respectful, person-centered, and culturally responsive social services that support dignity, independence, choice, and community participation.
Our objective is to reduce language, cultural, accessibility, and system-navigation barriers by connecting individuals and families with reliable information, community resources, and appropriate social-service supports.
