Know who does what
Three roles, different responsibilities.
CFSS can involve a participant, a consultation services provider, an FMS provider, a provider agency, and a lead agency. Knowing the lanes can make conversations less confusing.
Lead agency
The county, Tribal Nation, or other responsible lead agency—or a managed care organization in some situations—handles assessment and authorization responsibilities described by DHS.
Consultation services provider
Provides education, supports service delivery planning if desired, reviews plans, and offers ongoing support. The person selects this provider.
FMS provider
Supports financial management and employer-related tasks for people using the budget model, according to current CFSS requirements.
Connect And Care LLC is independent of Minnesota DHS and is not currently a DHS-contracted CFSS Consultation Services provider. We do not determine eligibility, authorize services, or write, review, submit, or approve official CFSS service delivery plans. Official Minnesota DHS information and the responsible lead agency control current program decisions.
