Who this is for

Learning for the people around the plan.

Training can meet people where they are, whether they are making decisions for themselves, supporting someone else, or helping a team serve people well.

Participants and families
Build a clearer understanding of CFSS choices, planning conversations, roles, and available resources.
CFSS workers
Strengthen practical, respectful approaches that keep the person’s preferences and direction visible.
Provider organizations
Support teams with supplemental learning about person-centered practice, communication, and CFSS context.
Community and human-services professionals
Make referrals and conversations easier with shared language, reliable resources, and clear role boundaries.

Participants, families, provider organizations, and community professionals can start with a general question about the training that would be useful.

Accessible learning

Information should be usable, not just available.

We adapt the way information is shared so people can participate in a way that fits their communication, culture, pace, and preferences.

  • Use appropriate interpreters when language access is needed or requested.
  • Use plain language and explain program terms without assuming prior knowledge.
  • Offer visual learning alongside spoken or written explanations.
  • Use teach-back to check whether the information was understood, not to test the person.
  • Provide translated materials when available.
  • Respect participant choice about pace, communication, supporters, and decisions.
A person’s English proficiency should not be used as a measure of their ability to make decisions, express preferences, direct services, or participate in planning. Appropriate language access supports meaningful participation.

Developing for the Future

DHS-contracted CFSS Consultation Services

Connect And Care LLC is not currently a DHS-contracted CFSS Consultation Services provider. This future service is separate from the five current education and training offerings described above.

Connect And Care training is supplemental and does not replace required Minnesota DHS/MHCP training, certification, enrollment, testing, orientation, or other provider requirements.

Nothing on this page indicates DHS approval, endorsement, authorization, contracting, or enrollment. Official Minnesota DHS guidance and the responsible lead agency or managed care organization remain the authorities for program decisions.
